States with largest Medicaid enrollment declines

With Medicaid redeterminations complete in most states, Medicaid enrollment has decreased by 14.3% from March 2023, according to KFF.

States began the process of redetermining the eligibility of Medicaid beneficiaries in April 2023. During the COVID-19 public health emergency, states were prohibited from removing any beneficiaries from the Medicaid program, regardless of their eligibility status. 

Though more than 35 million people were disenrolled from Medicaid during the redetermination process, enrollment in the program is higher than in 2020, according to KFF data published  Sept. 18. Enrollment among adults is 20% higher than in February 2020, while enrollment among children is only 5% higher. 

A handful of states have yet to finish the redetermination process. North Carolina will complete the process in November, and Alaska and the District of Columbia will finish the process sometime in 2025. 

New York is still determining a final date to complete its unwinding process, according to CMS. 

Here are the states with the largest enrollment declines since March 2023: 

  1. Colorado: -32.7% 
  2. Utah: -31.2% 
  3. Montana: -30.2% 
  4. Texas: -28.7% 
  5. Idaho: -28.6% 
  6. New Hampshire: -27.2% 
  7. Oklahoma: -25.7% 
  8. Arkansas: -25.2% 
  9. North Dakota: -23.4% 
  10. Florida: -22.3%
