Blue Cross and Blue Shield of Minnesota has struck a value-based agreement with tech-enabled rural health startup Homeward that aims to close care delivery gaps.
Starting in the spring, BCBS Medicare Advantage members residing in 24 counties outside of the Twin Cities area will have access to Homeward's care services, according to a Feb. 23 BCBS news release. Those include in-home visits, community-based visits, and technology-enabled clinical services such as telehealth.
Homeward will have a full-risk value-based arrangement, meaning payment for care is tied to improving health outcomes and reducing the total cost of care, according to the release.
Homeward and BCBS plan to expand the agreement to additional Minnesota counties over time.
